I would bid 4NT, followed by 5 over any bid by partner.

4NT presumably shows diamonds, so 5 is free to use as a slam try in hearts if you want. Without that agreement, I would think 4NT followed by 5 is the slam try with 5 natural; this way seems messier to me though. 5NT is surely pick a slam. The remaining hand type is a grand try with self-supporting clubs, so I guess that should be assigned to 5. Cherdano's approach does indeed look good.
