Hi,
the first question to answer is, if 4C created a forcing pass seq, this may
allow you to pass, telling your p, that you are not sure, what to do.
We are in the B/I forum, so mentioning FP may or may not be approbriate.
Since you have an ordinary hand, and you see duplication in clubs, I would
suggest to go for blood, i.e. X, ..., but I get those things wrong quite
often.

Its simple, since partner knows that 5♣ is a sacrifice, you can pass and tell him that he gets to decide and all the blame goes to him for doing the wrong decision.
Unless partner has a 5th diamond, this is a 6 keycard hand (♠Q, ♥AK, ♦AQ, ♣A). Depending on your range partner will have promised only 3, or at least 4. If he has promised 4 (12+ HCP) then you are safe at the 5 level and should bid it IMO. Otherwise pass or double.
