IMO underleading Axxx is never obvious, specially not with a 9. But it is very logical here.
♦10 seems terrible to me unless the 2NT bid promised an unbalanced hand.
Given that this is a problem I would lead a spade, but at the table I might just lead a heart without second thought

Fluffy, it's it quite possible that the diamond layout is something like KJxxx vs xx, where it's important to be able to lead potentially 3 times from my hand? Have to lead the T to make that work.

One thing you learn after a while is you stop worrying so much about the
final outcome of a hand as long as you and your p both agree you did the
best you could do. In this case we have at least a semi unbalanced lho
that preferred a 1d bid (to a 1h bid if they had 4 hearts) so the likelihood
of lho holding 4h is minimal. rho bid 1s over 1d so them having 4h is
also miniscule.
A heart lead looks like a huge favorite to score at least 2 hearts for your side
with no guarantees of hitting p with any other lead. Even if a heart is wrong
(and there are times it will be this is bridge not chess we do lots of things
based in % rather than knowledge) dont worry about it:)))))

It's IMPs, not MP. While a heart lead will almost certainly produces 2 tricks for us, where are our other 3 tricks coming from? In fact, on some layouts a heart lead actually blows a trick ... imagine KJT in declarers hand, which isn't that unlikely. 2NT should show a good heart holding.

I would lead a spade, despite the general undesirability of leading a suit they have bid. On this hand they have bid so many of them!
I am not attacking, but leading as passively as I can: it is not as if partner is going to have a long establishable suit with side entries. I am hoping he has three good spades, or at least good enough spades to prevent me from blowing up the suit.
I would rate the club jack ahead of any red-suit lead.

From the bidding and my hand, I think that our best hope is that they have the unusual 7577 fit, with hearts being our best chance to set. It might be that they have split honors, and if so, hope RHO has just 2 and partner has KJxx and an outside winner.
They also might be 7478 and we can run 5 heart tricks.
