If you pass and bid 3♥ after (2♥) - pass - (2♠) I suppose thats OK too, but if LHO has 3-3 or better in the majors you might have to bid 4.

Of course this requires you to have the agreemnet that 4♥ doesn't show many HCP.
Do not underestimate the power of the dark side. Or the ninth trumph.

This was wild -
The bidding at our table went
(2D*)-P-(P)-2S
(P)-4H-(P)-4NT
(P)-P-(P)
We've now agreed that 4♥ is natural

This went one off after east led the K♠ and partner mis-guessed the end position

That's an unnecessarily kind way of putting it.
This is quite interesting as a play-or-defend problem. Should 4NT always make on a spade lead?

It is!
I think it should make.
With 5 tricks remaining, west has ♦AKx ♣Kx.
East must keep 3 diamonds to avoid an endplay against west, ♣JT to guard the clubs, and a spade. Impossible!

Why can't east pitch to Jx of diamonds?

Diamond towards the queen endplays west to lead from the ♣K.
